You can fill out this template as is, or use it as a guide to write your own plan. You can print copies to share with your support people, or keep a copy with you so that you and your support people can get you back on track when you’re not at your best. It’s a good idea to discuss the plan with your doctor and support people beforehand to make sure everyone is prepared.
Here’s an example of how you might fill out the plan, but you can make it work however is most useful for you and your situation. You might have one page for all your symptoms, or you might have a separate page for each depending on how much detail you’d like to add.
